By accessing and using FormPay.us ("Service"), you accept and agree to be bound by the terms and provision of this agreement. If you do not agree to abide by the above, please do not use this service.
Permission is granted to temporarily access and use FormPay.us for personal and commercial purposes. This is the grant of a license, not a transfer of title, and under this license you may not:
When you create an account with us, you must provide accurate, complete, and current information at all times. Failure to do so constitutes a breach of the Terms, which may result in immediate termination of your account.
You are responsible for safeguarding the password that you use to access the Service and for any activities or actions under your password. You agree not to disclose your password to any third party.
Some parts of the Service are billed on a subscription basis. You will be billed in advance on a recurring and periodic basis. Billing cycles are set on a monthly basis.
A valid payment method, including credit card, is required to process payment for your subscription. You must provide accurate and complete billing information including full name, address, state, zip code, and valid payment method information.
FormPay.us, in its sole discretion and at any time, may modify subscription fees for the Subscriptions. Any subscription fee change will become effective at the end of the then-current billing cycle.
We will provide you with reasonable prior notice of any change in subscription fees to give you an opportunity to terminate your subscription before such change becomes effective.
We issue refunds for subscriptions within 7 days of the original purchase of the subscription. Refunds are issued to the original payment method.
Our Service allows you to create, store, and share content, including form data and submissions. You retain all rights to your content. By using our Service, you grant us the right to store and process your content to provide the Service to you.
You are solely responsible for your content and the consequences of posting or publishing it. We do not endorse any content or any opinion, recommendation, or advice expressed therein.
You may not use FormPay.us:
We may terminate or suspend your account immediately, without prior notice or liability, for any reason whatsoever, including without limitation if you breach the Terms.
Upon termination, your right to use the Service will immediately cease. If you wish to terminate your account, you may simply discontinue using the Service.
In no event shall FormPay.us, nor its directors, employees, partners, agents, suppliers, or affiliates, be liable for any indirect, incidental, special, consequential or punitive damages, including without limitation, loss of profits, data, use, goodwill, or other intangible losses, resulting from your access to or use of or inability to access or use the Service.
Your use of the Service is at your sole risk. The Service is provided on an "AS IS" and "AS AVAILABLE" basis. The Service is provided without warranties of any kind, whether express or implied.
These Terms shall be governed and construed in accordance with the laws of the United States, without regard to its conflict of law provisions.
We reserve the right, at our sole discretion, to modify or replace these Terms at any time. If a revision is material, we will try to provide at least 30 days notice prior to any new terms taking effect.
